Cota is an accredited not-for-profit organization that has been providing community-based support to people living with mental health and cognitive challenges for over 50 years. At present, our wide array of services delivers much needed support to over 4,000 people/year who live with mental health challenges, psychogeriatric mental health conditions, acquired brain injuries, developmental disabilities, dual diagnoses and/or the experience of homelessness. These services include, but are not limited to, case management, supportive housing and adult day program services. Role Overview

The Case Manager, Mental Health (with addictions experience) will contribute to the development of comprehensive client-centered assessments and treatment plans, provide community-based case management supports to an assigned group of clients and also assume responsibilities for coordinating and/or facilitating client group sessions. This case manager will have specific expertise enabling them to provide education, information and support to promote hope and the pursuit of personal recovery.

About the Team

The Toronto East Integrated Service Team is presently seeking one (1) experienced, dynamic and motivated Case Manager to join our interdisciplinary program. This team is comprised of Case Managers who have expertise in Acquired Brain Injury, Mental Health, Dual Diagnosis, Geriatric Mental Health, and Addictions.

Responsibilities:

  • Conduct a minimum of 2 to 3 face-to-face visits per day with clients at their locations, including but not limited to: home, shelters and hospitals.
  • Interact directly with referral source, physicians, and other services involved in client’s care.
  • Initiate services to clients at the point of intake to ensure that proper assessment and initial service response occurs.
  • Develop coordinated care plans with/ for clients and complete the Ontario Common Assessment of Need (OCAN) and other assessments as required.
  • Collaborate with other systems, organizations and individuals from courts, probation, short-term residential beds program, hospital and community to ensure a well-coordinated service system.
  • Record client status/progress using the appropriate documentation system.

Qualifications:

Education & Experience

  • A combination of a post-secondary degree or diploma in a relevant field with, 2 years of direct experience in the delivery of community mental health & addiction services.
  • Experience in working within a multidisciplinary team.
  • Experience with group facilitation is considered an asset.
  • Experience working in a community agency an asset.

Knowledge & Skills:

  • Sound clinical knowledge and understanding of the client population and demonstrated ability to work independently and effectively with individuals with serious mental illness, substance use challenges, and complex needs.
  • Extensive knowledge of community services and resources relevant to client population will be considered an asset.
  • Demonstrated ability to provide holistic and comprehensive case management services to all clients including initial and ongoing assessments, goal setting, short and long-term case plan development, crisis intervention, progress monitoring, individual money management.
  • Knowledge of principles of case management theory and practice.
  • Demonstrated ability to use problem-solving techniques to identify and assess Service Users needs and develop appropriate intervention strategies. 
  • Demonstrated ability to engage in a meaningful relationship with a broad range of Service Users, including those who are resistant.
  • Skillful in assertive outreach and follow-up using a flexible approach to support/assist Service Users in remaining engaged.
  • Excellent knowledge of community resources and demonstrated ability to negotiate with others on behalf of Service Users.
  • Excellent communication and interpersonal skills.
  • Excellent clinical judgment and decision-making skills including a keen awareness of client-clinician boundary issues. 
  • Knowledge of and commitment to an Anti-Racism Anti-Oppression framework as well as an ability to articulate and implement Recovery Philosophies. 
  • Proficient in MS Office, and OCAN documentation.

Other:

  • Conduct visits in the Service Users' homes, hostels, hospitals and other community settings working in sometimes difficult conditions, including hoarding.
  • Comfortable working with a broad range of client groups.

Upon successful completion of a three-month period of employment, permanent full-time and permanent part-time staff working more than 20 hours per week will be eligible for employer-paid group insurance benefits. These employer-paid benefits will include:
